Navigating Care Systems

The information here is a starting point to help you explore family care topics. We encourage you to do additional research, including reviewing and comparing resources as needed. Note that the organizations and resources listed that are outside of U of T are not endorsed by the Family Care Office and are not exhaustive.

 

Resources within the Community

Home Care 

In Ontario, publicly-funded home care falls under the jurisdiction of the Ministry of Health (MOH). Local government-funded health services are planned and funded by Ontario Health atHome (formerly Home & Community Care Support Services). If a loved one requires home care, the first step is to contact them.

  • Elizz provides family-funded caregiver support and home care services.  
  • Living Assistance Services is a home health care service provider for senior citizens 
  • Victorian Order of Nurses is a not-for-profit, national health organization and registered charity offering a wide range of community health care solutions. 
  • Sprint Senior Care Accredited, not-for-profit community support service agency in Toronto providing practical and low-cost services to seniors and their caregivers. 
  • Circle of Care The goal of this agency is to make daily living easier, simpler and more meaningful. Circle of Care provides social work, home support, respite care and volunteer services.

  • Respite care information from the Province of Ontario  
  • Hospice Palliative Care Ontario is a provincial association of hospices and palliative care providers, professionals, and volunteers throughout Ontario. 
  • Philip Aziz Centre is a non-profit hospice providing respite, practical, emotional, spiritual and bereavement care for adults and children living with life-limiting illnesses and support to their loved ones, in the comfort of their own homes.
